My Premise Health Patient Portal
To sign up:  Please click the button below to generate an email to Premise Health support and provide your return phone number including area code. A team member from Premise Health support will call you to complete your patient portal registration. My Premise Health support is available Available Monday - Friday, 6 a.m. CST - 7 p.m. CST.
Why is a phone call required for registration?  For security purposes and the protection of your confidential, personal health information you will need to speak with a Premise Health support team member to authenticate and validate your identity. Once completed, a registration link will be sent to you via text or email to complete the online registration process.
