Annual Physical. An annual exam is essential to good health. The Health for Life Centers make it easy and convenient to check this off your to-do list and it is available to you at no cost. What makes it even better is that our providers will spend the time needed and talk with you to thoroughly evaluate your health. If appropriate, our Health for Life Center's concierge will arrange follow-up appointments at the Health for Life Centers or with a specialist in the community.
Chronic Condition Management. Being proactive can be the best medicine to help you control a chronic condition. The Health for Life Center’s condition management and monitoring programs are available to help you manage your chronic conditions. If necessary, our staff will work with you to set goals that can help you develop habits to take better control of your health.
Cost of Care. For employees and their family members enrolled in a High Deductible Health Plan (HDHP), the services you receive will be billed at a rate comparable to other providers in your area. You may use your Health Savings Account (HSA)* to pay for the services or reimburse yourself for your out-of-pocket expenses..
